State of West Virginia Wood County [?]
I Barnu Powell a notary Public of said county and state certify that on this 9th day of July D. 1866, personally appeared before me, John Snodgrass, who being by me duly sworn, according to law, says he is the individual John Snodgrass that served as a private in Captain Cumberland Adkins company of Home Guards, Virginia state militia in Boone County in said state during the years 1861 and 1862 in safepressing the rebellious of 1861.
And I hereby acknowledge I have received of Colonel Geo. W. Brown Quartermaster General of the state of West Virginia the sum of one hundred and sixty-dollar in full for said services.

Sworn before me and [illegible] in my presance[sic] and I certify I am well acquainted with John Snodgrass and am satisfied that he is the person he represents himself to be.
